Level 2 Physiotherapist – Community Aged Care
Part time 28 hours per week (Monday, Tuesday, Thursday, Friday)

Join our dedicated team delivering physiotherapy services to older adults under the Commonwealth Home Support Programme (CHSP).
This community-based role involves providing one-on-one clinical interventions and leading group exercise programs to support clients’ independence and wellbeing.
We’re looking for a motivated, AHPRA-registered physiotherapist with at least 2 years' experience, strong communication skills, and a passion for helping older people live safely at home.

Position Summary
The Level 2 Physiotherapist will deliver high-quality physiotherapy services to older adults within the Commonwealth Home Support Programme (CHSP). This includes assessment, clinical intervention, and development of individual and group-based exercise programs tailored to promote independence, function, and safety in the home and community.
The role involves working collaboratively with clients, carers, and a multidisciplinary team to support clients to age in place safely and improve their quality of life.
Key Responsibilities and Duties
Clinical Duties:
• Conduct comprehensive physiotherapy assessments in clients’ homes or community settings.
• Develop and implement individualised treatment plans based on assessment outcomes and client goals.
• Provide one-on-one clinical interventions focused on functional improvement, falls prevention, pain management, and mobility.
• Design, lead, and evaluate group exercise programs in community settings.
• Monitor client progress and modify interventions as required.
• Promote safe and independent living by providing education, equipment prescription, and home exercise programs.
• Document all clinical activities in accordance with organisational and CHSP requirements.
Collaboration and Communication:
• Liaise with clients, carers, GPs, and other healthcare providers to ensure coordinated care.
• Participate in multidisciplinary case discussions and care planning.
• Provide input into program development and quality improvement initiatives.
Administrative and Compliance:
• Maintain accurate and timely clinical records using electronic systems.
• Ensure services meet CHSP guidelines, funding body requirements, and organisational policies.
• Adhere to professional, legal, and ethical standards of practice.

Essential Criteria
Required Qualifications
• Tertiary qualification in Physiotherapy.
• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
• Minimum 2 years post-graduate experience in a community, aged care, or rehabilitation setting.
Education and Experience
• Experience in delivering group-based exercise programs.
• Understanding of wellness and reablement frameworks.
Skills and Competencies
• Strong understanding of falls prevention, chronic disease management, and healthy ageing principles.
• Demonstrated ability to work autonomously and as part of a multidisciplinary team.
• Excellent interpersonal, verbal, and written communication skills.
Certifications and Licenses
This position requires you to have:
A current NDIS Worker screen
Current driver’s licence and access to a reliable vehicle.
A current first aid certificate
Desirable Criteria
• Experience working with CHSP-funded clients.
• Training in manual handling, aged care assessments (e.g., FROP-Com, SPPB), or similar tools.
