Belong Blue Mountains can provide the support you need to stay at home.  From domestic assistance to friendly visitors, social groups, grab rails and home maintenance. 

 COVID-19 UPDATE: Due to government restrictions, our Volunteer Visitor services have temporarily changed to non face-to-face forms of contact and support including phone and video calls.

    HEAL is a program aimed at building individual capacity and resilience. 

    Our team of health professional work to support wellness, re-ablement and restorative care for older people in our community.  

  •  ASSIST: HOME BUILDING 

     & MAINTENANCE SERVICE 

    ASSIST Building and Maintenance supports older people and people with a disability to stay safe at home. From simple garden maintenance and light bulbs changes to major home modifications, ASSIST can help.

  •  BLUE MOUNTAINS   PALLIATIVE SUPPORT 

    Blue Mountains Palliative Support Service is a free volunteer-based service that provides support to people who are terminally ill in their home, in hospital and in Aged Care, and to their carers and families

     VISITORS PROGRAMS 

    Our Community Visitor Scheme & Volunteer Home Visitors Services have changed to a phone and video service for now due to COVID-19.  But please contact us to see how we can help.

  •  COMMUNITY SUPPORT 

     DOMESTIC ASSISTANCE 

    Through our Community Support Program we provide domestic assistance including housework, cleaning and shopping to help you maintain your independence at home.

  •  COMMUNITY SUPPORT 

     TRANSPORT 

    Through our Community Support Program we can arrange transport with our trained volunteers if you need to attend a social group, event or other support program or activity. 

  WHO CAN ACCESS THESE SERVICES?  

Blue Mountains Palliative Support is available free of charge to Blue Mountains and Lithgow residents with a life-limiting illness. Referrals for this service can be made to us by community nurses, general practitioners and community services.

 

HEAL Services are available free of charge to people 65 years and over residing in the Blue Mountains and Lithgow areas, however eligibility may be dependent upon current services received and provided by other commonwealth program streams. 

ASSIST Building & Maintenance Services are available if you have a My Aged Care referral (subsidised fees apply), are an older person living at home without a My Aged Care referral (full fees apply), or if you have an NDIS plan.

Community Visitors Scheme is a free service available for people living in Aged Care Facilities, or people in their own home receiving support with a Home Care Package.

Volunteer Home Visitors, Domestic Assistance and Transport services are available for people with a My Aged Care referral, who don't have either a Home Care Package or an NDIS Plan.

You don't need a referral before you contact us.  If you would like help to determine what you are eligible for and how to apply for support, please contact us.  We can provide you with more information and support you through the process.

  GROUPS & ACTIVITIES  

We normally have a host of social and support groups, activities and community events, however due to COVID-19 we have had to suspend these for now.  We are however running some of our social and support groups in our Virtual Community Room via Zoom and Teleconference.  So please contact us and follow us on Facebook if you are interested in joining or running a virtual group.

  INFORMATION & REFERRALS TO OTHER SERVICES  

Finding the right help and support can be a difficult and sometimes overwhelming process.  If we don't have the support you need, we can provide you with information about other services and connect you with people who can help.  So please don't hesitate to give us a call or drop in for a chat about what you need.
